HR Company and Supervisor Evaluation Survey
Measures how employees rate the organization overall and their direct supervisor specifically, across culture, fairness, feedback, and growth — with an AI follow-up that digs into the concrete moments behind a supervisor's rating instead of settling for a number.

Probe the reasoning behind the respondent's satisfaction rating of their direct supervisor: ask them to describe a specific recent interaction that shaped that rating, whether it involved feedback, recognition, fairness, or support. If the rating was low, find out what a better version of that interaction would have looked like; if it was high, find out what the supervisor does that others could learn from. Also connect this to their top max-diff pick by asking how their supervisor could realistically act on it.
Is there anything specific you'd like your supervisor or company leadership to do differently? A concrete example is helpful.
Which department or team are you part of? (Template note: replace with your own department list before launching.)

How it compares
We reviewed the closest templates from other survey tools. Here’s what they do well — and where this template goes further.
Why this template
- Pairs an organization-wide culture/fairness matrix with a separate supervisor-specific matrix and satisfaction score, so one flow captures both company-level and manager-level signals.
- Includes an AI follow-up interview that probes the concrete reasoning behind each employee's supervisor satisfaction rating instead of settling for a bare number.
- Uses a MaxDiff question to force-rank which changes would most improve day-to-day work experience, plus an open-ended prompt for specific asks of supervisors or leadership.
- Captures tenure, department, and recent feedback frequency alongside the ratings, giving built-in segmentation without custom setup.
QuestionPro
21 Employee Feedback Survey Questions + Sample Questionnaire TemplateThis page is primarily a guide/blog listing 21 sample employee feedback questions, with a template embedded rather than a standalone fielding-ready survey. It's a reasonable question bank for building an org/supervisor survey manually, backed by an established enterprise survey platform.
What it does well
- Broad, well-organized question bank covering multiple employee feedback dimensions
- Backed by a mature survey platform with extensive question-type and reporting support
- Useful as reference content for building a custom questionnaire
Where it falls short
- Static, fixed question list with no adaptive follow-up that digs into the reasoning behind a rating
- Presented as blog/guide content mixed with template, so it's not a ready-to-send survey out of the box
- No AI-driven interview or automated per-response quality scoring

Typeform

- Widely recognized format that respondents are familiar with
Where it falls short
- Static form with no adaptive AI interview to dig deeper into a given rating
