Virtual Workplace Relationships & Team Connection Survey
Measures how well employees build and sustain real working relationships with managers and coworkers through online and hybrid communication channels. Built for People teams assessing remote/hybrid culture, with an AI follow-up interview that surfaces a specific moment where an online interaction built — or broke — a work relationship, beyond what a connection score alone shows.

Which best describes how you currently work?
- Fully remote
- Hybrid (mix of remote and in-office)
- Fully in-office, but I work with remote colleagues
- Fully in-office with no remote colleagues
Based mainly on your online or virtual interactions, how strong does your working relationship feel with each of these groups?

- Your immediate team members
- Colleagues in other departments
- Senior leadership
Overall, how connected do you feel to your coworkers given how much of your interaction with them happens online?
In a typical week, how often do you have non-work conversations with colleagues over chat, video, or other online channels?
- Never
- Rarely (once or less)
- Sometimes (2-3 times)
- Often (almost daily)
Which of these get most in the way of building real relationships with coworkers online, and which matter least?
- Lack of a video/camera-on culture
- No informal chat channels or virtual water-cooler time
- Time zone differences
- Too many meetings, not enough casual time
- Difficulty reading tone or intent in text
- Underused or clunky collaboration tools
- Infrequent in-person meetups
- Manager doesn't prioritize team bonding
How would you rate the tools your company provides for staying connected with colleagues (chat, video calls, shared spaces)?
Have you ever met your current teammates in person?
- Yes, all of them
- Yes, some of them
- No, none of them
- Not applicable — I work fully in-office
Probe the story behind the respondent's connection score: ask for one specific recent moment where an online interaction (a chat thread, video call, or its absence) either strengthened or damaged a work relationship, and what happened next. If they flagged a barrier like time zones, unclear tone, or lack of casual time, dig into how that barrier actually played out in a real interaction rather than in general terms. If their connection score was high, ask what specifically their manager or team does online that makes it work, so it can be identified as a repeatable practice.

Why this template
What this template is built to do — we found no directly comparable template from other survey tools to review.
What sets it apart
- Includes an AI follow-up interview that probes the story behind each respondent's connection score, asking for one specific real interaction that built or broke a work relationship — something a static rating scale can't surface.
- Combines quantitative measures (slider matrix on relationship strength, opinion scale on overall connectedness, rating of company connection tools, max-diff on top barriers) with qualitative depth in a single flow.
- Purpose-built for People teams assessing remote/hybrid culture specifically, with items on work mode, in-person contact history, and frequency of non-work conversation — not a generic relationship survey.
- Produces an automated report plus transparent, reviewable AI prompts, so People teams can see exactly how the follow-up question was asked and quality-score each response.
