Hair Consultation Intake & Goals Survey
Captures a client's hair history, concerns, and desired outcome before a cut, color, or style service, with an AI follow-up interview that turns vague inspiration ('like this photo, but...') into a concrete brief a stylist can actually execute. Built for salons, barbershops, and independent stylists running consultations.

If you have a reference photo of the look you want (or a photo of your current hair), upload it here. Optional but very helpful for your stylist.
Build a concrete stylist brief from the client's stated goal and any reference photo. Ask what specifically they like about the reference (color depth, cut shape, texture, length) versus their own hair, and what has gone wrong in past services when a stylist tried to achieve something similar. If their goal description is vague or purely emotional ('I just want a change'), probe concrete anchors: recent length, last color they loved, and one thing they never want to happen again. Also surface any deal-breakers (e.g., won't go shorter than X, no bleach) before they walk into the chair.

How it compares
We reviewed the closest templates from other survey tools. Here’s what they do well — and where this template goes further.
Why this template
- Includes a long-text goal description plus a photo response so clients can upload reference images before the appointment
- Pairs that input with an AI follow-up interview that turns a vague reference photo or phrase into a concrete, executable stylist brief
- Uses a slider matrix and multiple-choice concern/budget questions to quantify priorities and constraints up front, not just capture free text
- Ends with a satisfaction rating and booking-likelihood opinion scale so the salon gets a signal on conversion before the client even sits in the chair
Jotform
Monat Hair Consultation Quiz Form TemplateA static, brand-oriented quiz form built around Monat's product line rather than a general salon service intake. It's fielding-ready as a simple form but is oriented toward product recommendation, not building a stylist's execution brief. No mechanism to clarify an ambiguous client goal beyond fixed answer choices.
What it does well
- Ready-to-use, no-code form builder with drag-and-drop customization
- Branded quiz format may feel engaging for product-focused consultations
- Part of a mature, widely-used form platform with integrations
Where it falls short
- Fixed quiz questions only — no adaptive follow-up to clarify a vague style goal or photo
- No AI interview or voice option to turn a reference photo into a concrete brief
- No published methodology or per-response quality scoring
SurveySparrow
Hair Extension Consultation Form Template: Enhance Client ExperienceA conversational-style static form template scoped specifically to hair extensions rather than general cut/color/style consultations. It's a usable starting template but relies entirely on predefined questions with no dynamic probing of client answers. Good for a narrow extensions use case, less so for broader salon intake.
What it does well
- Conversational one-at-a-time question flow can feel friendlier than a long static form
- Purpose-built for the extensions niche with relevant field choices
- Part of a broader survey platform with reporting/dashboard features
Where it falls short
- No adaptive AI interview to probe vague answers or interpret reference photos
- Scope limited to extensions, not a general cut/color/style consultation brief
- No transparent prompt methodology or automated per-response quality scoring
Typeform
Free Hair Consultation Form TemplateA general-purpose, free static hair consultation form using Typeform's familiar one-question-at-a-time interface. It's fielding-ready out of the box but is limited to fixed question paths with no follow-up logic beyond simple branching. No photo-to-brief translation or AI-driven clarification of client intent.
What it does well
- Clean, well-known conversational form UX with high completion rates
- Free to use and easy to customize for salons of any size
- Simple logic/branching available for basic personalization
Where it falls short
- No adaptive AI follow-up interview to turn vague goals or photos into an actionable stylist brief
- No voice AI interview or screen-share guided task option
- No automated quality scoring or auto-generated consultation report
