Industry Knowledge Check & Training Confidence Survey
A multiple-choice knowledge assessment for industry-specific training, certification prep, or compliance programs — checks whether staff can correctly apply role-relevant rules and scenarios, then uses an AI follow-up to reconstruct the reasoning behind wrong or low-confidence answers so trainers can fix the actual gap, not just the test score.

(Replace with a second industry-specific scenario question, e.g. 'Which of these situations requires immediate escalation?')
- (Replace with correct answer)
- (Replace with plausible wrong answer 1)
- (Replace with plausible wrong answer 2)

Overall, how confident do you feel applying this material on the job right now?
Walk the respondent back through the scenario question they seemed least sure about or answered incorrectly. Ask them to explain, in their own words, how they reasoned through it and why they chose that answer. If their confidence rating was low despite correct answers, probe what specifically makes them feel unsure. If they got everything right and rated confidence high, ask for a real example from their own work where this knowledge actually applied, to check the answer isn't just test-taking pattern-matching.
Which topic areas would you like more training or examples on? (Select all that apply)
- (Replace with topic A)
- (Replace with topic B)
- (Replace with topic C)
- (Replace with topic D)
How clear was the training material this check is based on?
How long have you worked in this role?
- Less than 6 months
- 6 months to 2 years
- 2 to 5 years
- More than 5 years
- Prefer not to say
Which team or department are you part of?
- (Replace with team A)
- (Replace with team B)
- (Replace with team C)
- Prefer not to say
All done — thanks for your honesty! Your answers (and the follow-up conversation) go straight to your training team to sharpen the material where it's genuinely confusing, not just where scores were low.

How it compares
We reviewed the closest templates from other survey tools. Here’s what they do well — and where this template goes further.
Why this template
- Includes multiple industry-scenario multiple-choice questions designed for role-specific training, certification, or compliance checks, not generic quiz content
- Adds an opinion-scale confidence check so trainers see not just right/wrong answers but self-assessed readiness to apply the material
- Uses an AI follow-up interview that walks the respondent back through the scenario they were least sure about, reconstructing their actual reasoning rather than just recording a wrong answer
- Closes with a multi-select 'topics you want more training on' question plus a rating of the training material itself, so results map directly to what trainers should fix next
Jotform
Multiple Choice Test TemplateA ready-to-use static test template built on Jotform's form builder, suitable for quick knowledge checks. It supports standard multiple-choice question layouts and Jotform's usual scoring/reporting add-ons, but it's a generic test shell rather than one built around industry-specific scenario reasoning.
What it does well
- Fielding-ready template within a mature, widely-used form builder
- Likely supports basic auto-scoring and export via Jotform's broader ecosystem
- Easy to customize question wording and options
Where it falls short
- Static multiple-choice format with no adaptive follow-up to explore why an answer was wrong
- No mechanism to capture respondent confidence or reconstruct reasoning behind mistakes
- No published methodology for how scoring or question logic works
SurveySparrow
Multiple Choice Questionnaire Template for Quick SurveysA conversational-style multiple-choice questionnaire template aimed at quick, general-purpose surveys rather than training or compliance assessment specifically. It benefits from SurveySparrow's chat-like UI, but the template itself is generic and would need heavy rebuilding for scenario-based industry knowledge checks.
What it does well
- Conversational, chat-style question flow that may feel more engaging than a plain form
- Fielding-ready template usable out of the box for basic multiple-choice surveys
- Part of a broader survey platform with reporting dashboards
Where it falls short
- No adaptive AI probing into low-confidence or incorrect responses
- Not purpose-built for industry scenario or compliance-style questions
- No transparent documentation of question logic or scoring methodology
Typeform
Multiple Choice Quiz TemplateA polished, fielding-ready quiz template using Typeform's signature one-question-at-a-time interface, well suited for lightweight knowledge quizzes. It's a static quiz format, however, with no built-in path for follow-up questioning tied to training or compliance gaps.
What it does well
- Clean, well-known conversational UI that tends to improve completion rates
- Fielding-ready template requiring minimal setup
- Good branching/logic options within Typeform's standard feature set
Where it falls short
- No adaptive AI interview or voice follow-up to probe reasoning behind wrong or unsure answers
- Not tailored to industry-specific scenario or compliance training content
- No automated reasoning-focused report generation beyond standard quiz scoring
