Podcast Listener Feedback & Retention Survey
Captures how listeners actually consume your podcast — ideal episode length, topic preferences, ad tolerance, and what would push them to subscribe or share it — with an AI follow-up that digs into the real reason behind their loyalty score instead of a generic satisfaction number.

How much do you agree with each statement about ads on podcasts?
- I skip through ads whenever I can

- No ads at all, even if it costs more
- No strong preference
How likely are you to recommend this podcast to a friend or colleague?
Probe the reasoning behind the recommendation score the respondent just gave. If it's a 6 or below, find the specific moment or aspect (length, topic, ad load, host, pacing) that would need to change to raise the score, and connect it back to their earlier ad-tolerance and length answers if relevant. If it's a 9 or 10, uncover the one thing that most makes them an advocate and whether they've actually shared or recommended it, not just felt positive about it.
Rank these in order of what would most make you subscribe permanently or share this podcast with someone else.
- Shorter, more consistent episode lengths
- More episodes on topics I care about
- Fewer or shorter ad breaks
- A guest or host I already follow
- A recommendation from a friend or community
- Bonus content or an ad-free tier
Anything else that would make you listen more often or tell others about the show?

How it compares
We reviewed the closest templates from other survey tools. Here’s what they do well — and where this template goes further.
Why this template
- Includes a best-worst trade-off question that forces trade-off ranking of topics respondents want more/less of, not just a flat list
- Uses an AI follow-up interview step that adaptively probes the specific reason behind each respondent's recommendation score, instead of stopping at a number
- Combines a matrix on ad-attitude statements with a separate multiple-choice on preferred ad format, capturing both sentiment and format tolerance
- Ends with a ranking question that orders concrete incentives (length, content, guests, etc.) by what would actually drive a permanent subscribe or share
Jotform
Podcast Listener Survey Form TemplateA genuine, directly comparable podcast listener feedback template on a mainstream form-builder platform. It's a static, fieldable form rather than an interview experience, so all questions are fixed at design time with no dynamic branching based on a respondent's specific answers. Good for basic listener demographics and preferences, but not built to explore the 'why' behind any single score.
What it does well
- Simple drag-and-drop customization typical of Jotform's builder
- Free tier available and easy to embed or share via link
- Large template library and integrations (e.g., email, spreadsheets) for basic distribution and collection
Where it falls short
- No adaptive AI follow-up — every respondent sees the same fixed question set regardless of their answers
- No automated per-response quality scoring or AI-generated analysis of open-text answers
- No voice AI interview or guided screen-share task option; text/click responses only
