Event Speaker Bio, AV & Travel Intake Questionnaire
Collects everything organizers need from a confirmed speaker in one pass — bio, session title and abstract, AV/technical requirements, travel logistics, and photo/video consent — for anyone producing a conference, meetup, or panel. An AI follow-up catches AV mismatches before showtime by probing exactly what will happen on stage.
Sample questions
A preview of what’s in the template. Every question is editable before you launch.
Full name, exactly as it should appear on the program and badge.
Professional title and organization/company.
Please share a short bio for the program (100-150 words, third person). Include what you do and why you're speaking on this topic.
Session title, exactly as it should appear in the schedule.
Session abstract/description for attendees (2-4 sentences). What will people learn or be able to do differently after your session?
Which format best describes your session?
- Keynote
- Panel discussion
- Breakout workshop
- Lightning talk / short talk
- Fireside chat / interview format
What AV or technical setup will you need on stage?
- Handheld microphone
- Lavalier/headset microphone
- Projector/screen for slides
- HDMI/USB-C adapter for my laptop
- Podium laptop instead of my own
- Internet/Wi-Fi access
- Live demo requiring internet or an app
- Video/audio clip playback
- Recording of my session
Probe the specifics behind the speaker's AV selections: what exactly happens on stage (a live demo, a video clip, audience polling, panel mic handoffs), what could realistically go wrong technically, and whether they're bringing any of their own equipment. Also ask them to state in one sentence what attendees will be able to do differently after the session, and check that it matches the AV they requested — if the abstract implies a live demo or multimedia but they selected minimal AV needs (or vice versa), flag the mismatch directly and ask them to resolve it.
What travel support will you need for this event?
- Flights need to be booked for me
- Hotel accommodation needed
- I'll drive or arrange my own travel
- I've already booked and will submit for reimbursement
If you need travel or a hotel arranged, what are your preferred arrival and departure dates? (Leave blank if not applicable.)
Any accessibility, dietary, or other accommodation needs we should plan for?
Photo, video, and recording consent
Thank you — you're all set! Our production and travel teams will use this to prep your AV setup and logistics; we'll follow up if anything needs clarifying before the event.

How it compares
We reviewed the closest templates from other survey tools. Here’s what they do well — and where this template goes further.
Why this template
- Combines bio, session title/abstract, AV/technical needs, travel logistics, and photo/video consent in a single intake pass instead of requiring multiple forms
- Includes a dedicated AI follow-up interview that probes the speaker's AV selections to catch mismatches (e.g., clicker vs. handheld mic, screen-share needs) before the event
- Structures session abstract and bio fields with word-count and formatting guidance (100-150 words, third person) so program copy is ready to use as submitted
- Captures travel support type plus preferred arrival/departure windows and accessibility/dietary needs in the same flow, so production and travel teams work from one source of truth
Typeform
Event Questionnaire TemplateThis is a general-purpose event questionnaire template, not one built specifically for speaker bio, AV, or travel intake. It's fielding-ready and benefits from Typeform's clean conversational form design, but organizers would need to heavily customize it to capture session abstracts, AV specifics, or travel logistics. Useful as a generic starting point rather than a purpose-built speaker intake tool.
What it does well
- Polished, mobile-friendly conversational form format
- Easy to customize question types and branching logic
- Established template library aimed at event use cases broadly
Where it falls short
- No speaker-specific fields for bio, session abstract, or AV/technical requirements out of the box
- Static question set with no adaptive follow-up to catch AV or logistics mismatches before showtime
- No automated per-response quality scoring or auto-generated production report
