No. PLACES values are model-based estimates that CDC produces from the Behavioral Risk Factor Surveillance System. They are not counts of patients or of survey responses in each area. CDC cautions against using them to evaluate local programs or to measure trends.

Does it include any patient or provider records?

No. Every row is an estimate for an area. There are no patient records and no provider directory such as NPI.

Coverage: 3,144 counties, 29,923 places, 83,522 census tracts and 32,520 ZIP Code Tabulation Areas in the 50 states and the District of Columbia. 2025 and 2024 releases. No Puerto Rico or territories. Kentucky and Pennsylvania have 5 measures only in the 2025 release.

Why pay for public data

CDC publishes PLACES as a separate file for each geography and each release, in a long layout with text-coded value types. This build types every column, keeps the 95 percent confidence limits, and adds a wide table per geography so that one row is one area.

Pull the 2025 and 2024 PLACES releases for all four geography levels.

Type every column, keep both confidence limits, and build long and wide tables for each level, with a measure catalog that says which release and data year each value comes from.

Values are model-based estimates, and the tables say so. Nothing is smoothed, filled or re-modeled. Rebuilt on each annual release.
