Coverage: About 120,000 providers of 18 types, about 82,000 of them active, in 58 states and territories: hospitals, nursing homes, home health agencies, hospices, ambulatory surgical centers, rural health clinics, FQHCs, dialysis facilities and more. About 680,000 CLIA laboratory certificates, about 300,000 of them active. One quarter, 2026-04-01 to 2026-06-30. Earlier quarters are not included. Terminated providers and labs are kept.

Why pay for public data

CMS publishes three CSV files in two formats, with cryptic column names and codes whose meaning changes by provider type. This package types every column, decodes the codes with the CMS record layouts for each provider category, puts every provider type in one table with one set of column names, and keeps every other published field, such as the services each hospital provides, in a documented long table.

How it is built

    Pull the latest quarter of the three CMS Provider of Services files from data.cms.gov: the QIES and iQIES facility files and the CLIA laboratory file, all for the same quarter.

    Type every column, decode QIES codes with the list for the row’s provider category, map iQIES provider types to the same categories, and put the other 292 non-empty fields in one row per provider and field.

  3. 3

    Drop fax numbers, and withhold names and addresses that look like a person, with the phone number beside them. Rebuilt quarterly, once all three CMS files for a new quarter are out.

Questions

Is this data not free from the government?
Yes. The source files are public and we link to them. You pay for the cleaning, the joins, the documented schema and the refresh, not for the data itself.
How is it delivered?
As Parquet or CSV files, by download link or to your own cloud bucket. The same tables load into Snowflake with the scripts we include.
Can I see the data before paying?
Yes. Ask for a sample and we send the Vermont rows: about 240 providers, 6,400 attribute values and 1,400 laboratories, plus the full data dictionary.
Does it include earlier quarters?
No. This is the Q2 2026 release, 2026-04-01 to 2026-06-30, and each refresh replaces the quarter. CMS publishes earlier quarters back to 2011 for the QIES and CLIA files and back to 2023 for iQIES. Terminated providers and labs are kept, so openings and closures can be read from their dates.
How current are beds, staff and services?
They are what CMS recorded at the last certification or survey, which can be years old for some providers. Use the certification dates to judge how current a row is.
Does it join to other CMS data?
Yes. CCN is text with leading zeros as CMS publishes it, so it joins to CMS facility identifiers in other CMS provider files. Hospitals list their CLIA numbers, and more than 99 percent of those, about 7,700, are in CLIA_LAB.
Does it include personal data?
No patient data, and fax numbers are dropped. Many physician office labs are registered under the doctor’s own name, so names that look like a person are withheld and flagged: about 93,000 lab names and 540 provider names. Addresses that name an apartment are withheld too, and so is the phone number wherever a name or address is. The screen is a conservative heuristic, so some business names are withheld as well.

The Provider of Services files are US government works published by CMS on data.cms.gov, in the public domain in the United States. CMS does not endorse this product.
